This young patient with -20 D in one eye and - 18 D in the other eye. Lasic cannot be done. Can we do clear lens extraction with IOL implantation rather than implantation of Phakic IOL. This is a controversy. Clear lens extraction is not liked by many. Their reason is that patient loses accommodation once for all. But we must remember that patient with high myopia usually do not use accommodation and they may not be able to afford Phakic IOLs. In such cases they may afford a monofocal IOL and postoperative progressive multifocal spectacle.
