Dr. Ebraheim’s educational animated video describes Femoral Head Pipkin fracture classifications. Pipkin fracture is a fracture of the femoral head and dislocation of the hip. There are four fracture types within the pipkin fracture classification Type I Posterior dislocation with femoral head fracture caudad to the fovea. Treatment Nondisplaced: conservative treatment Displaced: reduce and fix or excise the fragment use anterior (smith patersen) approach for better visualization. Type II Posterior dislocation with femoral head fractures cephalad to the fovea. Involves the weight bearing surface ORIF using Herbert screws Type III Posterior dislocation of associated femoral neck fracture High incidence of AVN in type III Open reduction of hip, neck and head. Closed reduction may displace the neck fracture Older patient: use prosthesis Type IV Fracture of the femoral head associated with acetabulum fracture Increased incidence of arthritis. Use appropriate treatment for femoral head and acetabulum. Trochanteric osteotomy may be needed in the treatment of this type of fracture Urgent reduction of the dislocation within 6 hours is essential. CT scan should be obtained after reduction of the hip to evaluate the presence of any loose bodies within the acetabulum and also to evaluate the displacement of the fracture.
