IOL reposition and then exchange by Steven G. Safran MD

This is a patient who is over a year out from cataract surgery with a 1 piece acrylic lens placed. One haptic is int he bag, the rest of the lens is in the sulcus and over 300 degrees of the capsular bag is fibrosed shut behind the IOL. There is a large YAG capsulotomy opening. The capsular remnant was reopened and the lens dialed back into the bag but then after discussing the situation with the patient on the table we decided to remove and replace this lens as it would have left the patient about 1 diopter hyperopic. The lens was cut and removed and a new, higher power 3 piece IOL was placed in the capsular remnant to complete the case.
