The skew test, also known as the cover test, is a simple test used to determine whether a patient’s acute onset and continuous dizziness is due to a neurological disorder such as a stroke. The test involves a dizzy patient focusing straight ahead on a fixed target while the clinician covers and uncovers each eye, observing for any vertical movement of the uncovered eye. Normally, the uncovered eye remains steady. However, if the eye moves up or down, this indicates a vertical skew deviation, which can suggest a serious brain disorder involving the brainstem or cerebellum instead of a more benign cause due to an inner ear vestibular problem. Keep in mind that the skew test has a very low sensitivity of around 15%, but high specificity of about 98%… which basically means that this test is more reliable at correctly identifying those dizzy patients without a neurological condition than those with one. But, when this test is combined with a head impulse and nystagmus test (HINTs), diagnosing a stroke can be as accurate as an MRI scan!
