The modified pre decemetic deep anterior lamellar keratoplasty(pd-DALK)for acute hydrops after keratoconus was performed along with the bubbling of anterior chamber in the department of ophthalmology khyber Teaching Hospital Peshawar Pakistan. The tear margin of descemet membrane remains wide apart and everted and modified pd-DALK will bring the tear margin closer and closer and good attachment. This technique will prevent the post hydrop scarring and postpound the need for penetrating keratoplasty and its dreadfull complication and save the patient own endothelium. The technique provide visual, optical,biomechanical,topographic, refractive and structural stability and avoid the need for contact lens. Now pd- DALK is a strong indication for acute hydrop as early as possible and should not be delayed. Any type of keratoplasty procedure should not be performed in acute hydrop after pellucide marginal degeneration. The results was found excellent and will be shared subsequently.
