Endoscopic thyroid surgery is transforming the approach to thyroid surgery. This podcast was created in conjunction with the American Thyroid Association (ATA). The goal is to discuss up-to-date diagnosis and management for a wide array of thyroid diseases. I'm your host, Kaniksha Desai, and today we have a fascinating topic lined up for you, which is endoscopic thyroid surgery, also known as the scarless thyroidectomy. Endoscopic thyroid surgery, which was first described in the literature around 1997, has been expanding over the last decade and is transforming how we approach thyroid surgery. It offers patients an alternative to the visible neck surgical scar, a concern for a large portion of our thyroid patients, especially younger and female patients. For today's podcast, we'll explore the technical aspects of the procedure, including different surgical approaches, then we'll talk about patient selection criteria as well as pre- and postoperative care. Lastly, we will be discussing using this endoscopic thyroid surgery option for both benign thyroid diseases and for cancer patients. To help us navigate this exciting topic, we have a distinguished guest joining us, Dr Leonardo Rangel from Rio de Janeiro State University, where he is chief head and neck surgeon. Many of you may already know him through his cutting-edge clinical work with radiofrequency ablation. You'll be excited to know that he's also an expert in endoscopic thyroid surgery and has been a pioneer in the field of thyroidology. Thank you so much for joining us today. Leonardo Guimarães Rangel, MD: Thank you so much, Dr Desai. Desai: If you can tell our listeners what exactly is endoscopic thyroid surgery, also known as scarless thyroidectomy. Rangel: We, surgeons, developed an eagerness to include thyroid surgery in the endoscopic benefits of laparoscopic equipment that we saw as benefitting other organs. Many different approaches were tried, used, and fell into disuse because these different approaches had different advantages and disadvantages and were hard to adopt. The latest one, the last one that we had, is transoral endoscopic thyroidectomy — scarless, as we say, because in the first techniques we tried to hide the scar to make it smaller. None of those had all the benefits that we have now with this scarless one, the one in which we don't have any scar apparent on the skin, not only on the neck. This was the latest approach, and we understand why it would be the last one because from all the other aspects, thyroid surgery is considered as a clean surgery. We try to avoid going through the mouth because we think about infection and potential problems. It took us a very long time to be courageous enough to use that pathway.
