Stages of COVID-19 Disease describes the body changes and immune system reactions that occur in response to COVID-19 infection. The COVID-19 virus enters the cells of the human body and takes over that cell’s normal function. The COVID-19 virus forces that cell to make more copies of the COVID-19 virus. To make more copies of the COVID-19 virus, that cell has to stop performing that cell’s normal function. The loss of normal cell function will lead to destruction of the organ to which that cell belongs. The destruction of that organ eventually leads to the death of that organ. When unable to stop the spread and a person becomes seriously ill, shift the focus to stopping the disease progression. If complications are not quickly recognized, diagnosed, and treated, the risk for death greatly increases. Most complications are due to inflammation – not the actual viral infection. The COVID-19 virus triggers a violent inflammatory response in the body called cytokine storm. Cytokine storm develops when the immune system turns ON to fight off the infection by (1) creating antibodies to COVID-19 to kill the virus and (2) releasing cytokines to fight off the infection but then doesn’t stop. The danger and problem occurs when the immune system does not turn off after the infection is no longer a threat to the body. When the immune system rages out of control and continues to release cytokines when the cytokines are no longer needed to fight off the infection, the cytokines begin the destroy vital organs in the patient’s body. If a cytokine storm is not quickly recognized, diagnosed, and treated – the vital organ damage worsens until the patient dies. It is best if cytokine storm is treated as early in the process as possible – before extensive vital organ damage develops. Medications called immune modulators can calm the immune system and stop the cytokine storm. CONSULT with a rheumatologist who has specific experience with treating cytokine storm. Tocilizumab and sarilumab are two immune modulators that have been helpful to stop the cytokine storm in some COVID-19 patients. Research studies are ongoing to determine the safety and efficacy to these promising medications. Photo-immunology Therapy The UVB light used in phototherapy can induce immunosuppression. Through a similar mechanism, phototherapy may be beneficial to COVID-19 patients impacted by cytokine storm with minimal risk to these patients.
