Dr. C has made it his mission to provide the proper, best and sole gynecomastia treatment to these men and free them from their prison cells. Upon inspection by Dr. C, he determines that it is a case with a large amount of gyno. The gynecomastia presents lateral fullness in Zones 0, 1, 2 and 3 and it is hard to determine whether it is “meaty” tissue or “stringy” fibroglandular tissue. As with any case where there is a large amount of gyno tissue, whether skin removal treatment should be used is a major consideration. Skin removal treatment is the removal of excess skin and repositioning of the nipple-areola complex. This is a much bigger procedure with a longer recovery, increased cost, and results in scars across the chest, under the arms, and around the areolas. Often, skin removal is not necessary, and the patient’s skin has the laxity to tighten up correctly. Be wary if you are told that skin removal is necessary, as many patients receive it when they really do not, resulting in higher costs and scars they didn’t need to receive. Other than the possibility of needing skin removal, Dr. C will perform a standard gynecomastia surgery treatment that includes VASER and traditional liposuction, gyno tissue removal, internal flaps, if needed, and contouring of the chest. What is essentially the entire procedure will be shown, as Dr. C covers several different aspects of gynecomastia surgery all packed into one video. As we get underway with the gynecomastia procedure, Dr. C administers a tumescent solution that consists of a numbing agent and epinephrine, which will help reduce bleeding, increase the effectiveness of the VASER liposuction, among many other benefits. This is followed by use of VASER ultrasonic liposuction to liquify as much fatty and gyno tissue as possible in the affected areas, reducing the amount of tissue that is needed to be removed excised manually. It is also less painful, produces less scarring, and reduces both bruising and swelling post operatively. After an initial round of VASER lipo, traditional liposuction is used to remove the liquefied tissue produced by the VASER and further remove as much tissue as possible. The remaining gynecomastia tissue is then excised, followed by some additional liposuction to sculpt the chest to produce a good, even look. Finally, internal flaps are made and sutures are placed.
