It typically occurs in older individuals and has a poor prognosis due to its aggressive behavior and resistance to treatment. Pathologically, anaplastic carcinoma of the thyroid is characterized by the presence of large, pleomorphic cells with frequent mitotic figures and areas of necrosis. The tumor cells have lost their normal thyroid cell characteristics and are no longer responsive to thyroid-stimulating hormone (TSH). Immunohistochemical studies of anaplastic carcinoma of the thyroid may show expression of epithelial markers such as cytokeratin and epithelial membrane antigen (EMA), as well as markers of mesenchymal differentiation such as vimentin and smooth muscle actin. Genetic alterations, including mutations in TP53, BRAF, and RAS genes, are commonly associated with anaplastic carcinoma of the thyroid. These mutations are thought to contribute to the aggressive nature of the tumor by promoting uncontrolled cell proliferation and inhibiting apoptosis. Treatment for anaplastic carcinoma of the thyroid typically involves surgical resection, although the extent of resection depends on the size and location of the tumor.
