22 years female presented with decrease of vision in both eyes. She was diagnosed to have PDR with Combined RD in both eyes. In left eye, the vision was no perception of light. In right eye, she had perception of light with accurate projection of rays. As the patient was one eyed, despite being at the end stage, the patient was advised surgery. She underwent Vitrectomy with membrane removal and Silicon Oil Injection. We used a new instrument ARROW for effective membrane dissection. Post operatively, the retina was attached and the patients vision improved gradually.
