Dr. Evan Carstensen, a board-certified orthopedic surgeon fellowship trained in sports medicine, discusses reverse total shoulder arthroplasty or replacement. This procedure is used in patients with rotator cuff arthropathy, where there is an irreparable rotator cuff tear, sometimes in the setting of arthritis. Reverse total shoulder arthroplasty is indicated to address pain in the shoulder for rotator cuff tears that cannot be effectively repaired. The replacement utilizes the deltoid muscle to permit range of motion, addressing patient pain by preventing bone-on-bone contact that causes shoulder blade pain. The procedure is typically performed on an outpatient basis, with patients going home the same day. It takes about an hour to two hours to perform with patients wearing a sling for about four weeks post-surgery. While there is some surgical pain, the procedure decreases pain, especially nighttime pain, and may improve function, in the majority of patients. Another option for shoulder arthroplasty is anatomic total shoulder arthroplasty. This procedure also utilizes a functioning rotator cuff to improve range of motion and decrease pain, particularly nighttime pain, in patients with general osteoarthritis.
