Halton Intermediate Care and Frailty Service

The Halton Intermediate Care and Frailty Service brings together health and care professionals from organisations across Halton to support frail people and their family carers, based on their needs. The aim of the service is to keep people in their own home wherever possible. When someone is at risk of needing to be admitted to hospital, a rapid response approach is used which means the person is usually seen within two hours or the same day and a care plan is put in place. Bringing in support from health and care services across Halton when needed, the team provides a person-centred approach, which focusses on improving the quality of life of Halton's frail residents in their own home. We spoke to Ruth, from Murdishaw, alongside Julie, her nurse, to find out about the difference the service has made to her.
