It connects the chest to the abdomen, and just above the point where the celiac artery branches off from the aorta, it is normally attached. In MALS, the ligament's origin is slightly lower or the celiac artery's origin is slightly higher, causing compression on the celiac artery and leading to various symptoms. By carefully examining the graphics, you can understand how the Median Archoid Ligament can compress the celiac artery. Normally, there is no compression, and the celiac artery supplies major blood vessels, including those leading to the spleen, stomach, and liver. However, if compression occurs, it can affect the blood supply to these organs, resulting in symptoms. While four major theories have been proposed to explain the symptoms, the exact reason remains uncertain. The theories suggest that the compression reduces blood supply, leading to symptoms like stomach pain. Alternatively, collateral vessels may form due to compression, causing reduced blood flow and resulting in symptoms. Another theory involves chronic inflammatory conditions affecting the nerves around the celiac artery, increasing pain sensitivity. The final theory suggests that chronic changes in the outer part of the artery, known as Adventitia, may cause pain. Median Archoid Ligament Syndrome is a rare disease, making it challenging to diagnose in many patients. The main symptom experienced by patients is pain, usually occurring in the upper abdomen after eating. This pain after eating can lead to a fear of eating and gradual weight loss. One of the challenges in diagnosis is that routine tests, such as evaluating ulcers, cardiac problems, or gallbladder stones, often yield no significant findings. Consequently, the underlying cause of the pain remains unknown. Additionally, due to its rarity, many radiologists, surgeons, and other stomach specialists struggle to arrive at a differential diagnosis, further complicating the diagnostic process. Specific tests for MALS include CT scans, which can reveal if the celiac artery is properly filled with blood from the aorta or if there is a kink in the artery. Color Doppler tests can measure blood flow speed in the artery. However, it is crucial to rule out alternative diagnoses for upper abdominal pain, as not all patients with compression of the celiac artery experience the same symptoms. The primary symptoms of MALS include upper abdominal pain, which can occur both with and without eating. Some patients experience pain after consuming heavy meals or engaging in physical activity. Nausea, vomiting, diarrhea, constipation, blood pressure issues, and pulse-related problems may also manifest. Physically examining patients may not reveal many signs, as the artery is deep in the stomach. However, doctors may detect a bruit (abnormal sound) through a stethoscope in some patients, although this is not specific to MALS. The treatment for MALS involves surgically releasing the median archoid ligament using laparoscopy. This procedure is minimally invasive and allows for quick recovery. However, accessing the large vessels in the back of the stomach can be challenging, so specialized centers with expertise in advanced laparoscopic operations are typically involved. Robotic surgery is also an option, offering advantages like 3D vision and potentially improved precision. If symptoms persist or reoccur after the operation, patients may undergo a stenting procedure or bypass surgery to address the celiac artery. However, it is crucial to release the median archoid ligament as the first step, as directly performing stenting or bypass surgery without addressing the ligament may not fully resolve the symptoms.
