So here’s how I inject the temples, as promised. It’s a simple technique where I use the 27 gage half inch needle provided and, after placing the needle on bone, inject retrograde as I withdraw, moving the entire time that product is coming out of the needle. I inject, raising these columns of filler, anywhere there is volume loss in the temple, whether anterior, posterior, inferior or superior. I also inject in the hairline. You can’t just inject a quarter of the temple and get good results. I assume that there are vessels everywhere I am injecting and I think that movement, not trying to guess where the vessels are located, keeps me safe. I never inject boluses and I don’t rely on techniques like aspiration that have been clearly and decisively shown in the literature to be unreliable. Relying on aspiration gives injectors a false sense of security. I think that safety means never injecting a large amount (more than 0.1cc) in any one location and constant movement makes sure I never do that. Do I get filler into the muscle? Absolutely. But over the 20 years I’ve been doing temples this way, I may have had nodules a couple of times and these were easily dissolved. This is also an efficient way of filling the temples. Because I’m injecting in many layers, the fill is not dependent on lifting the entire bulk of the muscle from below.
