Deep anterior lamellar keratoplasty (DALK) is a specialized form of corneal transplantation that only replaces the outer layers of the cornea. The modern form of this technique is relatively new considering corneal transplantation has been available for over 100 years. Some of the advantages of DALK include lower risk for rejection and the potential for greater stability of the eye after the procedure. Often, rejection of corneal transplants occurs when the body recognizes and reacts to the endothelium or the inner layers of a graft. With DALK, the endothelium is not replaced and the rate of rejection appears to be lower. Also, since this layer is not removed, the patient's own cornea is not completely removed and the intact inner part has been shown to be strong and resistant to impact. The discovery of Dua's layer, the new sixth layer of the cornea, was aided by observations during DALK .
