In this video, we shares pearls for performing topical phacoemulsification combined with transpupillary silicone oil removal through posterior continuous curvilinear capsulorhexis.Cataract surgery was performed under monitored topical anesthesia in this 44 years old patient. A superior corneal incision (at steep axis) was employed, balanced salt solution supplemented with adrenaline was used as irrigating fluid, and Viscoat (Alcon- sodium chondroitin sulphate/sodium hyaluronate) as viscoelastic. A continuous curvilinear capsulorhexis was achieved and hydrodissection was performed with a hydrodissecting canula. The phacoemulsification technique consisted of a standard phaco chop method. We used an AMO phacoemulsifier (Sovereign Compact with WhiteStar ICE/CASE) (Abbott Medical Optics, Santa Ana, CA, USA) with a 30° tip. The phacoemulsification settings were 30 ml/min aspiration rate, 300 mm Hg vacuum for the removal of fragments at an initial bottle height of 90 cm. These settings were altered if abnormal fluctuations in the anterior chamber depth were observed during phacoemulsification. A foldable hydrophobic acrylic implant (AMO Sensar) was implanted. A posterior capsulotomy was fashioned and silicone oil was aspirated with a Peyman cannula prior to the insertion of a lens implant. Silicone oil has been used successfully in vitreoretinal surgery for many years. But complications from the use of silicone oil as a long-acting internal tamponade may develop. They include glaucoma, keratopathy, oil emulsification, and cataract. For patients requiring silicone oil removal and cataract surgery, both operations can be performed in a combined procedure. After phacoemulsification, a posterior continuous curvilinear capsulorhexis(PCCC) is performed, followed by irrigation of balanced salt solution (BSS) in to vitreous cavity allowing flow out of silicone oil through the PCCC and intraocular lens (IOL) is implanted into remaining capsular bag. This technique offers faster visual rehabilitation, shorter operation time and reduces the risk of peripheral retinal defect, vitreous hemorrhage that may be caused by traditional oil removal through sclerostomy.
