RetinaRounds™ 2: Post-op macular fold

Post-operative macular folds following retinal detachment repair can occur in patients with retinal detachments that transect the macula. Risk factors include superior retinal detachment, residual subretinal fluid, incomplete gas fill of the posterior segment, lack of face-down positioning in the immediate/early post-operative period and the presence of a scleral buckle. Patients with macular folds may experience blurred vision and metamorphosia. While mild folds can relax over time, more severe folds may need surgical intervention. In this case, we demonstrate a technique for repairing macular folds that involves peeling of the ILM, injection of BSS via subretinal cannula into the fold to dissect apart the adhesions of the inner retina, flattening of the macula with perfluorocarbon liquid, complete drainage of subretinal fluid, complete gas fill of the posterior segment and post-operative face-down positioning.
