Dr. Patrick S.t Pierre discusses how to control your pain after shoulder surgery. Every surgery has some component of pain associated with the surgical intervention to repair the shoulder, but we would like to minimize this as much as possible to make the experience more comfortable. A large component of this pain control is for you to start by taking two extra strength Tylenol tablets four times a day on the day prior to your scheduled surgery. We want you to take two pills as soon as you wake up, two at lunch, two at dinner, and two before you go to bed. This is the maximum dose of Tylenol, but we plan to only use it for about 5 to 7 days until you get through the postoperative pain. You will also receive two Tylenol tablets when you come in on the day of your surgery. Additionally, you will get a nerve block from an anesthesiologist on the day of surgery. This will provide complete analgesia to your arm for 18 to 24 hours. When you wake up from surgery, it is expected that you will have minimal to no pain, but we want to continue the Tylenol on the same program with two extra strength Tylenol tablets at lunchtime, dinner time and before you go to bed when you get home - even if you do not have pain. By taking two days of full strength Tylenol prior to experiencing pain, allows you to get ahead of the pain and will smooth out your transition when the block wears off. We recommend that you continue taking Tylenol for at least five days after the surgery, but then you can wean off of the Tylenol as your pain improves. Cryotherapy, or icing your shoulder, is very important following the surgery to help reduce swelling and pain. We will put ice on your shoulder in the recovery room and want you to continue it almost continuously when you get home for the first 5 to 7 days. You will have the opportunity to purchase an ice machine, which is an igloo cooler attached to a wrap that goes around your shoulder. These provide continuous cold to reduce swelling, but some models also have a component that provides compression, which will additionally help with the swelling. If you do not wish to purchase this device, you must have cold gel packs at home that you will be able to switch out to keep the shoulder cold and prevent swelling. We suggest that you have at least 3 to 4 of these gel packs at home to allow time for them to freeze and have them available almost continuously following the surgery.
