If, however, the intervention is not done in a timely manner, the longstanding inflammation of the tear sac causes it to dilate and loose tone. When this happens, a simple probing and irrigation with silicone stent insertion are not effective, but a dacryocystorhinstomy (DCR) has to be performed. This operation can be done through a skin incision (external DCR) or through the nose (endonasal/endoscopic DCR). In the second option there is no visible incision and therefore no skin scar. During the operation, a communication is formed, between the tear sac (which is usually filled with puss) and the nasal cavity. In order to achieve this, part of the bony wall needs to be removed in a gentle fashion. At the end of surgery, a silicone stent is passed and left for about a month. After its removal, the tears can flow normally from the eye to the nose, through the newly formed channel, which prevents any future infections of the tear sac.
