This video shows Pyonephrosis with multiple renal stones and a large vesical stone. Pyonephrosis is a term used to an infection of the kidney with pus in the upper collecting system which can progress to obstruction. There is an accumulation of purulent debris and sediment in the renal pelvis. Pyonephrosis on ultrasound is seen as multiple echoes within a distended pelvicalyceal system and results from pus, debris or hemorrhage. Urothelial thickening is a fairly non-specific finding seen in infection Pyonephrosis is suspected when the clinical symptoms of fever and flank pain are combined with the radiologic evidence of urinary tract obstruction. Debris in the upper collecting system on imaging raises even more suspicion. Pyonephrosis on ultrasound is seen as multiple echoes within a distended pelvicalyceal system that develops from pus, debris or hemorrhage. Urothelial thickening is a fairly non-specific finding seen in infection, Vesicoureteric reflux (VUR), stone disease, and obstruction. Ultrasound usually shows dilatation of the pelvicalyceal system with some additional features: Echogenic debris in the collecting system: considered the most reliable sign, Fluid-fluid levels within the collecting system, incomplete ("dirty") shadows of gas in the collecting system may occasionally be seen. Pyonephrosis—pus in the renal pelvis—results from urinary tract obstruction in the presence of pyelonephritis. Purulent exudate (inflammatory cells, infectious organisms, and necrotic, sloughed (sluhft ) urothelium) collects in the hydronephrotic collecting system ("pus under pressure") and forms an abscess. Pyonephrosis is defined as infected hydronephrosis complicated by infectious destruction of the renal parenchyma or the presence of pus in the dilated pyelocaliceal ( Pus in the pelvicalyceal system ). Pyonephrosis—pus in the renal pelvis—results from urinary tract obstruction in the presence of pyelonephritis. Similar to an abscess, Pyonephrosis is typically associated with fever, chills, and flank pain, although some patients may be asymptomatic. Pyelonephritis is inflammation of the kidney, typically due to a bacterial infection. Symptoms most often include fever and flank tenderness. Other symptoms may include nausea, burning with urination, and frequent urination. Complications may include pus around the kidney, sepsis, or kidney failure. Two common laboratory tests are performed to diagnose kidney infections (pyelonephritis). A urine sample is examined under a microscope to determine if white and/or red blood cells are present. The urine is also sent to the lab to see if bacteria grow in a urine culture. Etiology of pyelonephritis?. The main cause of acute pyelonephritis is gram-negative bacteria, the most common being Escherichia coli. Other gram-negative bacteria that cause acute pyelonephritis include Proteus, Klebsiella, and Enterobacter. In most patients, the infecting organism will come from their fecal flora. A possible complication of acute pyelonephritis is chronic kidney disease. If the infection continues, the kidneys may be permanently damaged. Although rare, it's also possible for the infection to enter the bloodstream. This can result in a potentially deadly infection called sepsis. What is the difference between UTI and pyelonephritis? Urinary tract infection (UTI) refers to any infection of the urinary tract (i.e. anywhere from the kidneys to the bladder). A kidney infection is called pyelonephritis and is usually more severe than a bladder infection alone, which is called cystitis. Pyelonephritis usually affects one kidney only.
