In this video we'll go over a case of 28 year old man who injured / fractured his penis by hitting his partner's pelvic bone. We'll start off with a basic explanation on what is Corpus Spongiosum and where similar penile fractures / injuries usually happen, which is Corpora Cavernosa and Tunica Albuginea (we'll briefly present them as well). We'll then continue to the actual penile injury case, subsequent findings, progression, and treatment. To summarize, a 28 year old man experienced a penile fracture / injury after hitting his partner's pelvic bone during sex. That happens when a thrust is misplaced out of the vagina and goes sideways. The patient reported a popping sound and a subsequent swelling, even in his flaccid penis state. The doctors suspected the usual Corpora Cavernosa / Tunica Albuginea injury and made an ultrasound. However, no findings of injuries on those two were found. In fact, there was no findings of a penile fracture on injury at all, even on the Corpus Spongiosum. Except for a hypoechoic area (a mode dense penile tissue) under the Corpora Cavernosa. The specialists decided to go on with a penile surgery and find what is going on with a direct physical examination. When the operation itself had started, an intraoperative cavernosogram was performed and a filling defect was found to the right (as well as penile bending to the right). The urethra seemed intact, but there was a hematoma over the Corpus Spongiosum. Removing the hematoma revealed a severe penile injury (a tear) underneath. The specific case was of interest because it did not involve the usual Cavernosal Bodies and Tunica Albuginea (and the ligaments of the penis), but was isolated to Corpus Spongiosum, thus showed a possibility of a very specific penile injury spots (depending on the angle of the hit). We'll then continue with presenting the best options for treatment if one fractures his penis, which are a surgical intervention. However, even with such an intervention, one could still develop various issues afterwards, with the three most common conditions being Peyronie's Disease, Mondor's Disease, and Lymphangiosclerosis. In case one does develop such, a topical treatment may produce some satisfactory results and alleviate / heal the conditions.
