The liver has a bunch of peritoneal structures, ligaments, bare areas and whatnot. Some of the ligaments are from peritoneum, but some are from blood vessels. Do you know what foetal blood vessels are associated with the liver and close up after birth? Let's look at the falciform ligament and remind ourselves how it is made, and where it is. Then we can look for the round ligament of the liver, also known as the ligamentum teres, and see it run towards the portal veins. The round ligament of the liver was the umbilical vein, so it runs from the umbilicus to the portal vein and carried blood from the placenta. There is another vein that helps this blood from the umbilical vein bypass the liver. This was the ductus venosus and closes up to become the ligamentum venosum after birth. We can find this on the posterior surface of the liver too. This anatomy explains one of the portosystemic anastomoses - a link between blood passing back to the inferior vena cava via the liver and the portal vein, and blood passing back to the circulatory system in a more direct manner. In fibrosing liver diseases it can become harder for blood to pass through the liver, raising the pressure of blood in the portal vein. Blood will then begin to flow through portosystemic anastomoses to get back to the systemic circulation from the gastrointestinal tract. What does this mean for the round ligament of the liver?
