Flap striae are small wrinkles or folds in the cornea following LASIK, occur due to misalignment of the flap edge and can impair vision, requiring surgical correction. Management involves a 3-step approach: 1.⁠ ⁠Epithelial Debridement: •Intracameral adrenaline is injected for better visualization. •Alcohol is applied for 30 seconds (protecting the limbus), followed by washing with BSS. •Complete epithelial removal is performed with a 15-blade, ensuring debridement across the striae’s crest, trough, and flap edge. 2.⁠ ⁠Flap Hydration: The LASIK flap is separated using a flap spatula, and distilled water is instilled over and under the flap to hydrate it. 3.⁠ ⁠Flap Ironing: The flap is repositioned.A semi-wet Merocel sponge is used to smooth out striae, ensuring proper alignment and even edges followed by bandage contact lens (BCL) application for 1 week.
