Right Perilymph Fistula: Dizziness, Migraine Headaches and Cognitive Dysfunction

This young woman was initially thought to have a CT negative third window syndrome which was treated with round window reinforcement surgery. This was before cochlea-facial nerve dehiscence was recognized as a clinical entity producing third window syndrome. Recent review of her high-resolution temporal bone CT revealed that she has a right cochlea-facial nerve dehiscence. This 16 year old young woman had severe migraine headaches, gravitational receptor dysfunction type of vertigo (dizziness and disequilibrium), sound-induced dizziness and cognitive dysfunction. She was thought to have a psychiatric problem called a conversion disorder. Surgical repair resolved her problems. She was unable to attend high school, but now has been accepted to college and she will be attending college in the Fall.
