A pilonidal sinus is an abnormal cyst usually located near tailbone at the top of cleft between buttocks. It usually containing hairs along with other skin debris. It is thought to occur due to punctures caused by hairs of back and these hairs get inside the sinus. It is otherwise painless but if it gets infected, it can cause extreme pain as well. Pilonidal cysts most commonly occur in young men, and the problem has a tendency to recur. People who sit for prolonged periods of time, such as truck drivers, are at higher risk of developing a pilonidal cyst. If it becomes infected- Pain Reddening of the skin Pus discharge Foul smell Causes The exact cause of pilonidal cysts is not clear yet but it seems that most of these sinuses are caused by loose hairs that penetrate the skin in this area due to skin rubbing against skin, prolonged sitting, tight clothing, bicycling, long periods of sitting or similar factors. These factors force the hair into the skin. Since hair seems like a foreign material to skin once it enters it, body reacts to create a cyst around it to protect itself. Due to same reasons barbers, dog groomers and sheep shearers have developed pilonidal cysts in the skin between fingers. Who is at most risk Males Prolonged sitting Excessive hairs on back Young age Obesity Inactive lifestyle Complications If a chronically infected pilonidal cyst isn't treated properly, you may be at slightly increased risk of developing a type of skin cancer called squamous cell carcinoma. Prevention To help prevent pilonidal cysts, try to: Infection Increase in size Rarely - skin cancer Treatment Most established cases of Pilonidal Sinus will need some form of surgery. LASER PROCEDURE (SiLaT) Sinus Laser Therapy or Endoscopic pilonidal sinus treatment (EPSiT) - Laser is fast becoming the procedure of choice for pilonidal sinus as this does not involve big wounds on this sensitive area and leads to much better recovery with much less pain. Traditional Surgery - To cut the infected sinus and then opt for one of the two options- Leave the wound open. If the problem involves a large area then this option is preferred by many surgeons. The surgical wound is left open and packed with dressing to allow it to heal from the inside out. This requires a long healing time but usually has lower risk of recurrence. Close the wound with stitches. While the healing time is shorter with this option, there's a greater risk of recurrence. Some surgeons make the incision to the side of the cleft of the buttocks or use flap in an attempt to prevent recurrence. Prevention Keep the area clean and dry Keep back hair free if there are excess coarse hairs on back Lose weight if you are obese Avoid prolonged sitting #pilonidalsinus #buttockssurgery #pilonidalsinustreatment
