Features of the new instruments. 1. The pre-existing Enclavation Needle has sharp tips. The new Enclavation Hook and Spatula have round tips(0.4 mm in diameter, 27G needle). So the new instruments cause less iris trauma and atrophy from enclavation of the haptics of the retropupillary Artisan. 2. In case of using the Enclavation Needle, multiple pushes are required to have the iris enclavated sufficiently along with the retropupillary Artisan. But in case of using the Enclavation Hook and Spatula, only single pushes are enough. 3. The Enclavation Hook can also be used to dis-enclavate the haptics of the retropupillary Artisan. 4. The Enclavation Hook & Spatula are reusable instruments. Case 1: PMMA PCIOL dislocation. Make the frown incision & the scleral tunnel to reduce astigmatism. Remove the dislocated PMMA PCIOL. Introduce a new upside-down aphakic Artisan into the AC. Holding its optic with the Sekundo Implantation Forceps, push the Artisan through the pupil behind the iris. Consrict the pupil. Inject the OVD. To recognize the clip site easier, press the Artisan against iris from behind. Enclavate the temporal haptic with the Lee Enclavation Spatula thru the 23G MVR paracentesis. Encounter with difficulties in clipping the opposite nasal haptic with the Lee Enclavation Spatula. Use instead the Lee Enclavation Hook thru the 20G MVR side port. Remove the OVD. Case 2: Traumatic dislocation of the temporal haptic of the aphakic Artisan. Insert 25G needle into the vitreous cavity through the appropriate sclerotomy. Push the dislocated Artisan from behind thru the pupil into the AC. Hold the optic with the Artisan Implantation Forceps thru the 2.85 mm limbal incision. Dis-enclavate the nasal haptics with the Lee Enclavation Hook thru the 20G MVR side port. To avoid possible re-dislocation or iris perforation when refixating the Artisan lens at the previous enclavation site, the aphakic Artisan lens is rotated counterclockwise about 30 degrees. Here, rotation of both the operation microscope and the operator’s sitting chair is recommended for better surgeon ergonomics. Hold the Artisan optic with the Sekundo Implantation Forceps thru the newly made 3.0 mm limbal incision. Push the Artisan thru the pupil behind the iris. Constrict the pupil. Fill the OVD into the AC. To recognize the clip site easier, press the Artisan against the iris from behind. Clip both haptics with the Lee Enclavation Spatula at the fresh iris sites. Remove the OVD. The retropupillary Artisan surgery may be easier, safer and more effective if these two new instruments are used.
