The common name "sebaceous cyst" is a misnomer; these have nothing to do with sebaceous glands. They are actually epidermoid cysts (benign follicular cyst, infundibular type). The smelly material inside the cyst is actually keratin, not sebaceous secretion. When epidermoid cysts and other cysts rupture, they usually cause granulomas and brisk inflammation. This makes the cyst enlarge and become red and painful clinically. Sometimes this is mistaken for infection clinically. From my Panniculitis (inflammation in subcutis) review.
