This patient is monocular and has an opaque cataract that developed in the setting of chronic uveitis. This has cause extensive synechiae with the iris firmly adherent to the posterior capsule. The surgeon is unable to free these adhesions with a simple sweet motion, as is commonly performed, and instead, micro-scissors are used to carefully cut these fibrous bands. A pupil expansion device used and then the easy part of the surgery is actually the cataract removal. This is a beautifully performed surgery by an expert surgeon in a case where the stakes are very high.
