Consent given by patient for this video. As demonstrated by Dr. Vinnith Pullavarayar. This was a very interesting case of primary knee osteoarthrosis with a posterior cruciate ligament injury. Ms. Shenegha, a year 3 MBBS student presented a case on primary knee osteoarthrosis. The patient surprisingly claims that he feels his right knee 'giving away' - a typical presentation of cruciate ligament injury. As we performed a complete joint examination, a latent finding found its place in this video. This is a reason why drawer test should not be omitted even if you feel that it is not related to an osteoarthotic case. It turns out that the patient had a trauma to his right knee, resulting in a posterior cruciate ligament injury. Steps: (1) Inspect laterally for posterior sag sign. Omission of this step may result in a false positive anterior drawer test. (2) Sit on the patient's forefoot and ensure his hamstrings are relaxed. (3) Place your thumbs on the medial and lateral joint lines with the other 4 fingers wrapping the leg. (4) Give an anterior and posterior force and see if there is any translation.
