We begin by outlining the overall organization of the parasympathetic division of the autonomic nervous system, describing its craniosacral outflow from the brainstem and sacral spinal cord. We cover the pathways of parasympathetic preganglionic and postganglionic neurons, including the role of cranial nerves III, VII, IX, and X and pelvic splanchnic nerves in reaching target organs. We then explain the physiological effects of parasympathetic activation, emphasizing its role in promoting rest-and-digest functions. These include pupillary constriction via the oculomotor nerve, stimulation of salivary and lacrimal glands, increased gastrointestinal motility and secretions, bronchoconstriction, and regulation of urinary and reproductive functions. We highlight the importance of acetylcholine as the primary neurotransmitter at both the preganglionic and postganglionic synapses and the role of muscarinic receptors in mediating target organ responses. Finally, we connect these concepts to the integrated control of body systems, showing how parasympathetic activity counterbalances sympathetic stimulation to maintain homeostasis.
