A 35-year-old male patient who underwent jaw surgery at another hospital in June 2024. He was originally diagnosed with bimaxillary protrusion and had excessive maxillary setback, causing severe jaw recession, airway narrowing, and sleep-disordered breathing. Prof. Uiyong Lee (Chung-Ang University Hospital, Dept. of Oral & Maxillofacial Surgery) presents this revision case with maxillary advancement + CCW rotation + downgraft. ✦ Initial Surgery Problems: Excessive maxillary setback of 6.7mm + clockwise rotation Mandible retracted approximately 14.73mm → extremely recessed chin Airway narrowing → sleep-disordered breathing, frequent waking Malocclusion + 1mm maxillary deviation to the right Marionette lines + double chin developed Patient unable to maintain normal social life ✦ Revision Surgery Plan: Maxillary advancement: 6.5mm (restoring original position) Counter-clockwise rotation (CCW): 2.5mm Maxillary downgraft: 2mm (to increase upper tooth show) 1mm left shift of maxilla (asymmetry correction) Mandibular advancement: ~11mm + genioplasty Iliac bone graft to fill bone gaps ✦ Results: Marionette lines resolved Nasolabial folds improved Smooth jawline restored (no indentation) Upper-lower lip step corrected Neck contour normalized Airway restored → improved breathing function ⚠️ Key Lessons: When the mouth goes back, the chin goes back too Excessive rotation is the #1 reason for revision surgery For males, a recessed chin is particularly detrimental Balanced rotation and movement amounts are critical 📋 Timestamps: 0:00 Introduction — Purpose of this video XX:XX Patient Overview (35M, post-surgery complications) XX:XX Pre-op CT Analysis of Initial Surgery XX:XX CT Superimposition — Confirming Excessive Setback XX:XX Airway Changes + Forward Head Posture XX:XX Malocclusion & Maxillary Deviation XX:XX Revision Surgery Plan (Advancement + CCW + Downgraft) XX:XX Digital Surgical Planning Simulation XX:XX Pre vs Post-Op Bone Changes XX:XX Iliac Bone Graft Explanation XX:XX Soft Tissue Changes — Marionette Line Improvement XX:XX 45° View — Jawline & Neck Contour Changes XX:XX Key Takeaway — Dangers of Excessive Rotation ⚠️ This video is for educational purposes only and is not medical advice.
