Duplicate records continue to be a significant challenge in healthcare organizations, arising from user errors, system limitations, and operational inefficiencies. These issues compromise data quality, affecting patient safety and care. Despite efforts by health information professionals to implement workflows and processes to mitigate duplicate records, the problem persists, underscoring the need to examine the organizational factors contributing to it. In this session, we will share findings from a recent study identifying key operational factors—such as work shifts, staffing levels, and time of day—that influence data entry errors leading to duplicate records. By analyzing our data, we will highlight how these factors impact the creation of duplicate records within healthcare settings. Participants will gain insight into how operational elements like shifts and admission volume may contribute to these errors. Additionally, we will offer strategies for reducing duplicate records from an operational standpoint. These recommendations will focus on improving data quality, streamlining workflows, and enhancing patient safety. Ultimately, the session will provide health information professionals with actionable strategies to assess and improve organizational processes, reduce the occurrence of duplicate records, and strengthen data integrity.
