Phacodonesis is a surgical challenge with high risk for early intraoperative complications such as severe zonulolysis, posterior capsule tear, vitreous loss and drop of lens particles to the vitreous, or late complications of IOL subluxation. When the ophthalmic surgeon deals with mild to moderate phacodonesis he must support the capsular bag with devices such as hooks or CTR. When there is a severe phacodonesis, a fixation of the IOL is needed. In this video, the surgeon used iris hooks to support the capsular bag and after extract the nucleus, he inserted a CTR for more support. At the end of the surgery the IOL stable and well centered. The surgery was done by Dr. Ori Mahler, Israel.
