Treatment of heart disease in children | Dr Vishal Changela | A Man on A Mission |

Namaskar friends, my name is Dr. Vishal Changela and I am a paediatric cardiologist. Today we will talk about the treatment of heart disease in children. There are mainly three types of treatments: first with medicines, second with catheter-based procedures, and third with open-heart surgery. Firstly, I want to mention that some heart diseases do not require surgery and naturally get cured over time. For example, a small hole in the heart called VSD, especially if it is in a muscular location, it closes on its own and does not require any surgery. Time will take care of the problem. Second, treatment with medicines. Some problems do not require surgery and can be managed with medications. For instance, sometimes the heart's pumping ability becomes weak or the lung pressure increases. In such conditions, we give medications to children and they gradually recover. Rarely, if there is a major problem with pumping that does not improve with medicines, heart transplantation might be needed. Now, let's talk about catheter-based procedures. Using angiography techniques, we reach the heart through a vein in the leg. A small puncture is made, and then we reach the heart because all blood vessels provide a path to the heart. Upon reaching the heart, if there is a hole, we close it with a device. If a valve is not opening properly, we use a balloon to inflate and open it. Sometimes a pacemaker is needed, and we can place that too. We can treat various heart problems in children using these minimally invasive techniques. It involves a hospital stay of one or two days, no major cuts or stitches, making it a cosmetic-level surgery. After such operations, even if the child is seen without clothes, it will not be apparent that they have undergone heart surgery. Recovery is quick, and the child can return to normal activities without any pain. This is called pinhole heart surgery. I have performed many such heart surgeries in the last 14 years, and the children are living healthy lives. Finally, open-heart surgery, in which we open the chest. The paediatric heart surgeon cuts the chest bone and opens it to reach the heart. Then, the problem in the heart, such as a hole, is patched up with stitches around it. If there is a valve problem, it is repaired, or if the valve is too damaged, it is replaced with a new one. If there is extra tissue, it can be removed, and abnormal connections can be corrected. Various heart repairs can be done through open-heart surgery. In open-heart surgery, there is a scar in the center of the chest or sometimes on the side. The child stays in the hospital for about 10 days, though sometimes they are discharged in 3-4 days, depending on the problem and surgery. So, in paediatric heart disease treatment, we have pinhole surgery and open-heart surgery. Most children recover well after these surgeries, leading normal lives like us, and they can participate in sports. There are many misconceptions, superstitions, and fears about children's heart surgery in society. Let's come together to dispel these fears, superstitions, and misconceptions.
