It was sensational - watched by 23 million people and it made a star of its reporter, Martin Bashir. But an inquiry into how he got that exclusive has found he faked documents to get access to Diana and later lied about it. The report says an initial investigation by the BBC was 'flawed and woefully ineffective' and that the organisation fell short of its own high standards.
