Ambulance Stretcher || How to transfer a patient on spine board || Karaganda Medical University Simulation center of Karaganda Medical University, Kazakhstan. 🧠 Purpose To immobilize the spine and safely transfer the patient without causing further spinal cord injury. ⚕️ Preparation Equipment needed: Spine board (backboard) Cervical collar Head immobilizer or sandbags with straps Straps to secure chest, pelvis, and legs At least 4–5 trained rescuers 🚑 Steps for Transfer onto Spine Board Step 1: Scene Safety & Initial Assessment Ensure the scene is safe. Assess the patient’s airway, breathing, and circulation (ABCs). Stabilize the cervical spine immediately by manual in-line stabilization. Step 2: Apply Cervical Collar Maintain manual stabilization of the head and neck. Gently apply a cervical collar to prevent neck movement. Step 3: Positioning Rescuers At least four rescuers are ideal: Rescuer 1: At the head, maintaining spinal alignment and giving commands. Rescuer 2 & 3: At shoulders and hips on one side of the patient. Rescuer 4: At legs. Rescuer 5 (optional): Places or slides the spine board under the patient. Step 4: Log Roll Technique The leader (at the head) gives clear commands. Rescuers kneel on one side of the patient. On the command “Log roll on three”, roll the patient as one unit (shoulders, torso, hips, and legs move together). Inspect the back for injuries, bleeding, or deformities. Step 5: Position the Spine Board Slide the spine board beneath the patient, aligning it with the body’s midline. Step 6: Roll the Patient Back On the leader’s count, roll the patient back gently onto the board — again, as one unit. Ensure the head, neck, and spine remain aligned. Step 7: Center and Adjust Adjust the patient so they are centered on the board (not tilted or off-balance). Step 8: Immobilization Secure the patient using straps: Across chest, pelvis, and legs. Apply head blocks or sandbags on both sides of the head. Secure the forehead and chin with straps or tape (do not cover the airway). Step 9: Reassess Recheck airway, breathing, circulation, and neurological status. Ensure no movement of the spine occurs during reassessment. ⚠️ Key Safety Tips Never twist or bend the spine. Always move on the team leader’s count. Keep the neck and head in neutral alignment. If vomiting occurs, log roll the entire body as one unit to clear the airway.
