Last night in the emergency, we had to take a difficult decision for a patient who has been through far more than most. With a long history of multiple surgeries and a complex abdominal fistula, her condition had been chronically challenging. When she began showing signs of persistent tachycardia postoperatively, something that should never be ignored & we acted swiftly. A CT scan confirmed our concern: a small leak in the small bowel, and signs of localized infection. In such high-risk cases, where healing is already compromised, controlling sepsis takes absolute priority. We performed a double-barrel ileostomy, a temporary but necessary step to divert bowel contents and give the infected segment time to heal. In four weeks, when the inflammation subsides and the infection clears, we’ll plan the reversal. Until then, we watch over her closely, knowing that sometimes the hardest interventions are the ones that lead to the quiet beginnings of healing.
