This video is intended to illustrate diverse concepts: Firstly, the HoLEP can perform the task of treating any prostate, regardless of size. Secondly, it is appropriate to intervene with plenty frail patients underlying disease. Finally, the objective of the surgery, being understood as prostatic adenomatous tissue resection, is complete and can be practically definite in time. In addition, this video has been made to take some considerations about the technique of enucleation in large prostates in order to help making the learning curve less steep and easier to extend the HoLEP. For that reason, we present the prostate enucleation anticoagulated patient with atrial flutter, with external pacemaker and a history of post-surgical bleeding large entity IUC carrier and a 300cc prostate (90x82x74).
