In cataract surgery we are replacing the thick human crystalline lens (4.5 to 5 mm thick) with a thin IOL optic (1 mm thin) and this means that the capsular bag will need to shrink-wrap and contract to securely hole the new IOL. To center our IOLs (with a typical diameter of 6 mm) inside the large capsular bags (typical diameter of 9 to 10 mm), we have haptics which have some degree of compressibility. In very myopic eyes, the axial length is longer and also the anterior segment size is also increased including the corneal diameter but also the dimensions of the crystalline lens and capsular bag. When we place a three-piece IOL in the capsular bag in these large eyes, the haptic force against the capsular bag equator can induce wrinkles for the entire diameter of the optic. This causes patients to see streaks in their vision like a Maddox rod effect. In this video I show a technique to remove these wrinkles from the center of the optic and we also compare to using a single-piece IOL.
