(EN) El pan nuestro de cada día. Paciente de 35 años, sin tratamiento médico. Se fue a Turquía, a los 30 años, con un grupo de compañeros a realizarse un trasplante capilar. Desastre general (hemos operado a 2 de este grupo ya): ·Depleción donante ·Falta de cobertura ·Antinatural (múltiples en 1ª línea) ·Falta de estrategia a largo plazo. Se medica con finasteride diario y se emplaza a los 6 meses con la esperanza de que mejore algo en donante o receptora. TDA corta (230 cm2), pelo fino y baja capacidad de cobertura sobre todo en las zonas deplecionadas (parietal/occipital). Se planea estrategia de rescate con un mínimo de 3 cirugías, y bajo número de unidades por sesión (2000 + BHT) A los 6 meses de iniciar medicación en quirófano. Donante: - Temporal: 50 μm ; 81 UF/cm2; 187 pelos/cm2; 2,31 pelos/UF; CV de 9,35. - Parietal: 50 μm ; 64 UF/cm2; 121 pelos/cm2; 1,89 pelos/UF; CV de 6,05. Occipital: 50 μm ; 75 UF/cm2; 161 pelos/cm2; 2,15 pelos/UF; CV de 8,05. TDA de 230 cm2. TDC de 4970 grafts (a 2,12 pelos/graft). TDHC de 10.513 pelos. Se extraen 2919 grafts 2519 scalp + 400 BHT (5915 pelos + 451), a 2,35 pelos/grafts, para 59 cm2 de frontal (37-42 grafts/cm2). Punch: 0.8 mm. Grafts: I-288; II-1183; III-890; IV-135; V-5. Bht : I-349; II-8 La extracción en negativo nos permite extraer más unidades del cuero cabelludo de las esperadas (2500 grafts). Se utilizan las 400 de BHT en ambas zonas parietales. Se extraen múltiples de línea frontal. A los 5 meses resultado muy prometedor: frontal natural y aumentando cobertura y coronilla con mejoría debido a la medicación. We extracted 2919 grafts 2519 scalp + 400 BHT (5,915 hairs) to restore 59 cm2 in the frontal area (37-42 grafts/cm2) at 2.35 hairs per graft. Punch: 0,8 mm. Breakdown: I-288; II-1183; III-890; IV-135; V-5. Bht : I-349; II-8 Negative extraction allows us to extract more units from the scalp than expected (2500 grafts). The 400 BHT were used in both parietal areas. Front line plugs were withdrawn.
