Capsular and subcapsular fibrous bands are often seen in advanced cataracts. They can end up with intraoperative surprise events, which can be as serious as Posterior capsular dehiscence, vitreous loss and spillage of lens matter into vitreous. Mastering low flow, low vacuum and low IOP techniques, and continuously modifying the depth of anterior chamber during surgery enable the surgeon to carefully preserve the hyaloidal face and insert the planned IOL into the posterior chamber.
