설명Title Ruptured abdominal aorta during removal of Unexpected disconnectedAnsel guiding sheath at left EIA Abstract At other hospital, Unexpected fractured and disconnectedAnsel guiding sheath at left EIA during endovascular procedure to left SFA long segment CTO by crossover technique (Figure.1). During trying removal of the sheath, iliac bifurcation site ruptured due to excessive strong trauma (Figure.2) and the vital sign was getting worse. Unfortunately, the guide wire was removed accidentally and 5cm sized fractured sheath remained at left EIA (Figure.3) Because of urgent situation, we decided to do aorto-uniiliac bypass operation, Fortunately, we could do passage wire to left EIA Ansel sheath through left CFA, and snared the wire through right CFA. And then inserted dilator and sheath through right CFA and push the disconnected Ansel sheath to left CFA with retrograde direction (Figure.4-5). After that, aorto-uni-iliac bypass operation and Rt-(CFA)-to-Lt-(popliteal) bypass operation and embolization of left CIA were done successfully (Figure.6),
