In the PNETs/PNENs series, this is part 1 Part 1 - Insulinoma is a rare neuroendocrine tumour arising from the insulin-producing pancreas called beta cells. It is typically a small, benign tumour, but in some cases, it can be malignant. Insulinoma is usually solitary and is characterized by excessive production and secretion of insulin, leading to hypoglycaemia (low blood sugar levels). Clinical Presentation: Insulinomas are commonly associated with symptoms related to hypoglycaemia. Patients may experience episodes of confusion, weakness, palpitations, sweating, and hunger, which are relieved by the ingestion of food. These symptoms typically occur when blood glucose levels drop below normal ranges. Diagnosis: The diagnosis of insulinoma involves a combination of clinical suspicion, biochemical tests, and imaging studies. Fasting tests, such as a supervised 72-hour fast, can help confirm the presence of hypoglycaemia during the episodes. Imaging techniques like computed tomography (CT) scan, magnetic resonance imaging (MRI), or endoscopic ultrasound (EUS) are used to localize the tumour within the pancreas. Surgical Management: Surgical resection is the mainstay of treatment for insulinomas. The goal is to remove the tumour while preserving pancreatic function. Minimally invasive techniques, such as laparoscopic or robotic-assisted surgery, are often preferred, as they offer advantages such as reduced postoperative pain, shorter hospital stays, and faster recovery. In cases where the tumour cannot be localized or if the patient is not a surgical candidate, medical management may be considered. Intraoperative Challenges: During surgery, it is crucial to identify the insulinoma accurately and remove it while preserving the surrounding pancreatic tissue. Intraoperative ultrasound can be valuable in locating small tumours and guiding the surgical approach.
