Little manoeuvre to rescue a rhexis

This is a white cataract which presents a high risk of tear out of the capsulorhexis. The surgeon here uses vision blue to stain the capsule then elects to spiral the rhexis out gradually to avoid a tear out. This seems to be going well until the difficult sub-incisional rhexis is tackled, where the tear begins to go out towards the zonules. At this point, the surgeon correctly stops, refills the anterior chamber with viscoelastic and unfolds the flap to its original position. A Little manoeuvre is attempted but this fails to work as the forceps are being pulled towards the centre of the lens. When the surgeon then correctly pulls the tear edge directly back, this diverts course away from the zonules. This is a great example of how not to then how to correctly perform Mr Little’s rhexis rescue.
