Miliaria, is a common disorder of eccrine sweat glands, often occurring in conditions of increased heat and humidity. It is postulated to be caused by blockage of sweat ducts, which leads to leakage of sweat into the dermis and epidermis. This condition is commonly seen among newborns, although it may also affect adults. There are 3 types of miliaria. In miliaria crystallina, the ductal obstruction is more superficial, and it produces tiny, fragile, and clear vesicles, measuring 1 to 2 mm in diameter. They are often confluent, as you can see in this picture, and they do not cause any symptoms. In infants, they tend to occur on the head, neck, and upper trunk. And in adults, they usually occur on the trunk. In miliaria rubra, the obstruction is deeper within the epidermis. The lesions are uniform, small, erythematous papules, which cause intense itching. Unlike in miliaria crystallina, they do not become confluent. And there may be absent sweating in the unaffected skin as well. In miliaria profunda, the obstruction is more deeper, occurring at the junction between the dermis and epidermis. It gives rise to asymptomatic, skin-colored papules. There is absent sweating in the affected areas, and increased sweating in the unaffected areas. Two most common complications of miliaria are secondary infection, and heat intolerance. As far as the treatment is concerned, miliaria crystallina does not require any treatment because it is self-limiting. For miliaria rubra and miliaria profunda, topical lotions containing calamine, boric acid, or menthol, cold, wet to dry compresses, frequent showering with soap, topical steroids, and topical antibiotics are recommended. In addition, the following measures can be taken to prevent the occurrence and relieve the symptoms of miliaria. Avoiding excessive heat. Controlling fever if present. Removing occlusive clothing. Limiting activity to reduce sweating.
