A 60 year's old gentleman presented with obstructive jaundice in Square hospital Dhaka Bangladesh. Procedure: E.R. C. P. with Metal Stening of the CBD Indication: Pancreatic mass with obstructive jaundice Medication: Under sedation FINDINGS: PAPILLA: Looks normal with free bile flow seen. CHOLANGIOGRAM: Critical stricture is noted at the lower part of the billiary duct due to chlangiocarcinoma or Ca head of the pancreas. Papillotomy is done with needle papillotome. A 6cm covered metal billiary stent is implanted into the bile duct across the narrowed segment with establishment of free flow of bile. PANCREATOGRAM: Not attempted. COMMENTS: Findings are highly suggesive of cholangiocarcinoma / Ca. Head of pancreas. THERAPEUTIC INTERVENTION: Papillotomy and metal stenting of the CBD done.
