If untreated, the condition significantly increases the risk of severe infections and blood cancers such as myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML). The transplant has not only cured her condition but also helped her return to a normal life within three months. Led by Dr. Gopinathan, Consultant, Hemato-Oncology, the team carried out a half-matched (haploidentical) bone marrow transplant — a technically demanding procedure due to only 50% genetic compatibility with the donor. The case was further complicated by Janani’s lung disease (pulmonary alveolar proteinosis) and a severe non-tubercular mycobacterial infection. Despite these challenges, the team ensured successful engraftment and carefully managed chemotherapy to suit her fragile condition, ultimately leading to a full recovery. Dr. M.A. Raja, Director and Senior Consultant, Medical Oncology, noted that GATA2-related immunodeficiency is extremely rare in adults and difficult to diagnose early due to its variable presentation. “Approximately 70% of those with GATA2 mutations develop MDS or AML,” he said. “This case highlights the importance of timely bone marrow transplantation in preventing life-threatening complications and offering a cure for such rare disorders.”
