A 10 mm hypoechoic mass is noted in the right lobe of the thyroid. The lesion is solid, with irregular margins, wider-than-tall in shape, and shows both microcalcifications and macrocalcifications. There is no surrounding halo, and color Doppler imaging reveals increased internal vascularity. No abnormal cervical lymph nodes are seen. Fine needle aspiration cytology was performed but yielded inconclusive results. Based on ACR TI-RADS scoring, the lesion is classified as TR5 (highly suspicious). The most likely diagnosis is papillary thyroid carcinoma, although follicular neoplasm or benign calcified nodule are differential considerations. Given the features and inconclusive FNAC, repeat aspiration or surgical excision is advised.
