This is a patient who was operated several years ago for retinal detachment and at that time it was implanted a PMMA IOL sutured at sclera. This IOL fell in the posterior pole, by complete break of one of the former prolene sutures, while the other prolene suture was very loose, allowing the IOL to partially escape from it. Pars plana vitrectomy was not necessary now, because after a complete check of the retinal periphery, we noticed a clean posterior cavity, needing for no completion of the former vitrectomy. We proceed to IOL lifting and removal, then to implantation of an Akreos IOL (implanted unfolded, since we already had to enlarge the limbal incision, for the PMMA IOL explantation); we sutured the Akreos with Goretex sutures. We used the former two superior sclerostomies for the superior halves of the Goretex sutures (peformed on purpose, from the beginning, at 3 mm from the limbus). The video presents preoperative images showing lack of any IOL in the pupillary area, irido-donesis and a small piece of one of the PMMA IOL haptics, when the eye was looking downwards. The video also presents postoperative photos of the anterior segment and the UBM image in two perpendicular axis, showing a very good centraton of the sutured Akreos IOL.
