A large hemorrhagic cyst occupying almost the entire right lobe of the thyroid appears as a predominantly cystic lesion with ground-glass echoes, indicative of hemorrhagic content. Incomplete thin septae are noted within the cyst, with smooth margins and minimal or absent internal vascularity on color Doppler. Peripheral vascularity may reflect surrounding inflammation. Common causes include hemorrhagic thyroid cysts or colloid nodules with hemorrhagic transformation. Thyroid malignancy with cystic degeneration or a thyroid abscess should also be considered if suspicious features like thickened, irregular septae or systemic infection are present. Prognosis is favorable in benign cases, as hemorrhage typically resolves spontaneously. Management involves analgesics, ultrasound-guided aspiration for symptomatic relief or diagnosis, and follow-up imaging to confirm resolution. Suspicious cases require fine-needle aspiration cytology and possible surgical evaluation to rule out malignancy.
