Common causes include hemorrhagic thyroid cysts or colloid nodules with hemorrhagic transformation. Thyroid malignancy with cystic degeneration or a thyroid abscess should also be considered if suspicious features like thickened, irregular septae or systemic infection are present. Prognosis is favorable in benign cases, as hemorrhage typically resolves spontaneously. Management involves analgesics, ultrasound-guided aspiration for symptomatic relief or diagnosis, and follow-up imaging to confirm resolution. Suspicious cases require fine-needle aspiration cytology and possible surgical evaluation to rule out malignancy.
